OTs are uniquely positioned to evaluate an individual’s functioning and related care needs. We are trained to view individuals through a holistic lens which considers physical, cognitive and emotional functioning in the context of their home and community. We are skilled at in-home evaluation and understanding how daily routines and activities are impacted by injury and chronic health conditions. Our scope of practice includes many key components of a life care plan, including the need for attendant care, rehabilitation services, assistive equipment, home modifications, housekeeping and transportation.

For life care plans, the cost of each report will vary depending on the size of the medical file to be reviewed, the complexity of client injuries/disabilities, the size of the treating medical/rehabilitation team involved, and the location of the client. Abbreviated reports can be prepared to accommodate smaller budgets. Contact me to obtain a cost quotation that is tailored to your case and needs.

Depending on the complexity of the case, a life care plan can take between 4 to 8 weeks to complete. Consider making a referral at least 2 to 3 months before you require the final report.
Other reports listed can be completed in much shorter time frames. -
Assessments may be conducted in-person at the client’s home or via videoconference. Rosa uses a functional approach to evaluate a client’s abilities and impairments to identify physical, cognitive and psycho-emotional factors which are contributing to loss of function. Where possible, caregivers and treatment providers are contacted for collateral information.
